Re-Recurrence of Intracranial Aneurysm with Proximal Vascular Stenosis After Primary Clipping and Secondary Endovascular Embolization: A Case Report and Literature Review— Zhang 2019

A limited percentage of patients could experience intracranial aneurysm recurrences following either surgical clipping or endovascular embolization therapy, it was recognized. However, early recurrence within 1 month of treatment and recurrence have only sometimes been documented in the past. We describe a 46-year-old guy who was noncompliant with the prescribed therapies and had a history of smoking and hypertension. It was discovered that he had a ruptured middle cerebral artery aneurysm when he admitted himself to the hospital with right extremity paralysis and aphasia. Surgery to cut the aneurysm was successful in treating it. The patient experienced an intracranial hemorrhage and a recurrence aneurysm at the same site one month after receiving treatment. Endovascular embolization was utilized to treat the recurring aneurysm. At the follow-up appointment eight months following the therapy, angiography revealed a recurrence of the aneurysm along with proximal vascular stenosis. After initial successful treatment with surgical clipping and endovascular embolization, an intracranial aneurysm may return. Local vascular stenosis may play a role in the recurrence of an aneurysm in addition to the patient's medical history and aneurysm characteristics. For these patients, a close postoperative follow-up is necessary.
